“I always look sad, even when I’m happy.” This lovely patient felt that her appearance didn’t reflect how she felt and that she often looked sad, even when she was perfectly happy 😟 As we age, changes in the lower face, including loss of support from the fat pads, reduced collagen, bone remodelling and increased downward pull from the muscles around the mouth can cause the corners of the mouth to turn downwards, creating a tired, sad or unhappy appearance. Marionette lines are rarely caused by just one fold or crease alone. A big contributor is volume loss through the lower face particularly in the area I like to call the “bow tie” (the mental fat pad, perioral and prejowl areas).
